IntroductionTranscranial magnetic stimulation is a consolidated procedure for the treatment of depression, with several meta-analyses demonstrating its efficacy. Theta-burst stimulation is a modification of TMS with similar efficacy and shorter session duration. The geriatric population has many comorbidities and a high prevalence of depression, but few clinical trials are conducted specifically for this age group. TBS could be an option in this population, offering the advantages of few side effects and no pharmacological interactions. BackgroundECMO is a particularly scarce resource during the COVID-19 pandemic. Its allocation involves ethical considerations that may be different to usual times. There is limited pre-pandemic literature on the ethical factors that ECMO physicians consider during ECMO allocation. During the pandemic, there has been relatively little professional guidance specifically relating to ethics and ECMO allocation; although there has been active ethical debate about allocation of other critical care resources. We report the results of a small international exploratory survey of ECMO (...) clinicians’ views on different patient factors in ECMO decision-making prior to and during the COVID-19 pandemic. We then outline current ethical decision procedures and recommendations for rationing life-sustaining treatment during the COVID-19 pandemic, and examine the extent to which current guidelines for ECMO allocation (and reported practice) adhere to these ethical guidelines and recommendations.MethodsAn online survey was performed with responses recorded between mid May and mid August 2020. Participants (n = 48) were sourced from the ECMOCard study group—an international group of experts (n = 120) taking part in a prospective international study of ECMO and intensive care for patients during the COVID-19 pandemic. The survey compared the extent to which certain ethical factors involved in ECMO resource allocation were considered prior to and during the pandemic.ResultsWhen initiating ECMO during the pandemic, compared to usual times, participants reported giving more ethical weight to the benefit of ECMO to other patients not yet admitted as opposed to those already receiving ECMO, (p < 0.001). If a full unit were referred a good candidate for ECMO, participants were more likely during the pandemic to consider discontinuing ECMO from a current patient with low chance of survival (53% during pandemic vs. 33% prior p = 0.002). If the clinical team recommends that ECMO should cease, but family do not agree, the majority of participants indicated that they would continue treatment, both in usual circumstances (67%) and during the pandemic (56%).ConclusionsWe found differences during the COVID-19 pandemic in prioritisation of several ethical factors in the context of ECMO allocation. The ethical principles prioritised by survey participants were largely consistent with ECMO allocation guidelines, current ethical decision procedures and recommendations for allocation of life-sustaining treatment during the COVID-19 pandemic. Consequence is at the heart of logic, and an account of consequence offers a vital tool in the evaluation of arguments. This text presents what the authors term as 'logical pluralism' arguing that the notion of logical consequence doesn't pin down one deductive consequence relation; it allows for many of them.

Background The expressions of a “wish to hasten death” or “wish to die” raise ethical concerns and challenges. These expressions are related to ethical principles intertwined within the field of medical ethics, particularly in end-of-life care. Although some reviews were conducted about this topic, none of them provides an in-depth analysis of the meanings behind the “wish to hasten death/die” based specifically on the ethical principles of autonomy, dignity, and vulnerability. The aim of this review is to understand if and (...) how the meanings behind the “wish to hasten death/die” relate to and are interpreted in light of ethical principles in palliative care. Methods We conducted a meta-ethnographic review according to the PRISMA guidelines and aligned with Noblit and Hare’s framework. Searches were performed in three databases, Web of Science, PubMed, CINAHL, with no time restrictions. Original qualitative studies exploring the meanings given by patients, family caregivers and healthcare professionals in any context of palliative and end-of-life care were included. A narrative synthesis was undertaken. PROSPERO registration CRD42023360330. Results Out of 893 retrieved articles, 26 were included in the analysis, accounting for the meanings of a total of 2,398 participants. Several factors and meanings associated with the “wish to hasten death” and/or “wish to die” were identified and are mainly of a psychosocial and spiritual nature. The ethical principles of autonomy and dignity were the ones mostly associated with the “wish to hasten death”. Ethical principles were essentially inferred from the content of included articles, although not explicitly stated as bioethical principles. Conclusions This meta-ethnographic review shows a reduced number of qualitative studies on the “wish to hasten death” and/or “wish to die” explicitly stating ethical principles. This suggests a lack of bioethical reflection and reasoning in the empirical end-of-life literature and a lack of embedded ethics in clinical practice. There is a need for healthcare professionals to address these topics compassionately and ethically, taking into account the unique perspectives of patients and family members. More qualitative studies on the meanings behind a wish to hasten death, their ethical contours, ethical reasoning, and implications for clinical practice are needed. (shrink)

A good argument is one whose conclusions follow from its premises; its conclusions are consequences of its premises. But in what sense do conclusions follow from premises? What is it for a conclusion to be a consequence of premises? Those questions, in many respects, are at the heart of logic (as a philosophical discipline). Consider the following argument: 1. If we charge high fees for university, only the rich will enroll. We charge high fees for university. Therefore, only the rich (...) will enroll. There are many different things one can say about this argument, but many agree that if we do not equivocate (if the terms mean the same thing in the premises and the conclusion) then the argument is valid, that is, the conclusion follows deductively from the premises. This does not mean that the conclusion is true. Perhaps the premises are not true. However, if the premises are true, then the conclusion is also true, as a matter of logic. The Liar paradox raises foundational questions about logic, language, and truth (and semantic notions in general). A simple Liar sentence like 'This sentence is false' appears to be both true and false if it is either true or false. For if the sentence is true, then what it says is the case; but what it says is that it is false, hence it must be false. On the other hand, if the statement is false, then it is true, since it (...) says (only) that it is false. -/- How, then, should we classify Liar sentences? Are they true or false? A natural suggestion would be that Liars are neither true nor false; that is, they fall into a category beyond truth and falsity. This solution might resolve the initial problem, but it beckons the Liar's revenge. A sentence that says of itself only that it is false or beyond truth and falsity will, in effect, bring back the initial problem. Orthologic is defined by weakening the axioms and rules of inference of the classical propositional calculus. The resulting Lindenbaum-Tarski quotient algebra is an orthoimplication algebra which generalizes the author's implication algebra. The associated order structure is a semi-orthomodular lattice. The theory of orthomodular lattices is obtained by adjoining a falsity symbol to the underlying orthologic or a least element to the orthoimplication algebra.

Semantic and soritical paradoxes challenge entrenched, fundamental principles about language - principles about truth, denotation, quantification, and, among others, 'tolerance'. Study of the paradoxes helps us determine which logical principles are correct. This article presents and transcribes a newly identified John Locke manuscript – an index Locke made of Anton Deusing’s De motu cordis et sanguinis itemque de lacte ac nutrimento foetus in utero, dissertationes (Groningen, 1655). Deusing (1612–1666) was a polymath and medical eclectic with a scholastic predisposition who wrote numerous medical texts in the 1650s and 1660s. Locke owned and read several of these works, taking notes from them, and indexing at least two of them during his medical research at (...) Oxford in the mid-1660s. The recovery of this manuscript further underlines Locke’s own medical eclecticism at this time, highlighting the contrast between his views in this part of his career and the empiricism he later came to evince. This paper examines the role of ideology in the emergence of the empirical sciences and the evolution of philosophy. It argues that the orientation of the religious ideology, Christianity, at the epistemological and ontological levels was very instrumental in the emergence of the empirical sciences in the area dominated by the culture of the Western (Latin) church. This claim is demonstrated by an analysis of the theoretical and methodological orientation of pre-Christian Europe, the epistemological and other philo- sophical values sponsored (...) by Christianity, as well as a comparative analysis of other cultural regions where such philosophical values did not exist. The paper then explores the evolution of philosophy after the emergence of the empirical sciences. It points out that the progress of philosophy along rational lines and the generation of knowledge thereof is not equivalent to embracing the method of the empirical sciences (scientism), but rather is a rediscovery of the rational attitude bearing in mind the context in which it has to operate. We are pluralists about logical consequence [1]. We hold that there is more than one sense in which arguments may be deductively valid, that these senses are equally good, and equally deserving of the name deductive validity. Our pluralism starts with our analysis of consequence. This analysis of consequence is not idiosyncratic. We agree with Richard Jeffrey, and with many other philosophers of logic about how logical consequence is to be defined. To quote Jeffrey.

The two purposes of this essay. The general philosophical problem with most versions of social libertarianism and how this essay will proceed. The specific problem with liberty explained by a thought-experiment. The positive and abstract theory of interpersonal liberty-in-itself as ‘the absence of interpersonal initiated constraints on want-satisfaction’, for short ‘no initiated impositions’. The individualistic liberty-maximisation theory solves the problems of clashes, defences, and rectifications without entailing interpersonal utility comparisons or libertarian consequentialism. The practical implications of instantiating liberty: three rules (...) of liberty-in-practice, 1) initial ultimate control of one’s body, 2) initial ultimate control of one’s used resources, 3) consensual interpersonal interactions and resource transfers. These rules are economically efficient and maximise general want-satisfaction. Private property and legal remedies are additional practical institutional aspects, but to which ‘initiated impositions’ then apply prima facie. Libertarian law is often mistaken for complete libertarianism. Moral explanations are a separate issue. The three main moral theories imply libertarianism, but it can be morally posited independently of them. Critical rationalism and its application. No empirical or argumentative support for theories. An important ambiguity with ‘justification’. How the epistemology applies to the theory of liberty and its application but remains separable in principle.
